IV. Statement of Purpose/Purposes of Portfolio - this page shall clearly state the purpose of the portfolio. It will express your intention in making the porfolio.

V. Organization of the Portfolio Entries (Journals)
Each section shall contain the journal title, entries of evidences and reflections. The organization may be sequential, categorical or thematic. Presentation of the entries will depend on your creativity. a. journal manual fully answered by the students

b. entries of evidence (narratives, reports, photos, essays, drawings, poems, pictures, clippings, reflections, checklist, and the like)
