client appears to be friendly and cooperative. She was happy because she got late passes for the New Years.

SOCIAL UPDATE: client report she does have family members here in NY but she prefer to live on her own. She also report she can wait to be around family members for the New Years.

RESOURCE UPDATE; Client has an active PA case.

Client report she is still waiting for SSI disbursement entitlement which it's pending.

SAVING; client has no money saved

LEGAL UPDATE: She mentioned she hired a SSI/SSD lawyer to assist her with the process

MEDICAL UPDATE: Client suffers from uncontrolled seizure disorder and she reports no seizures this week. nt. Client continues to take the following medications: Kepra 1000 ½ mg, Acetazolamide ER 500mg and Lamictal 25mg and refuse to have staff monitor her medications.

MENTAL HEALTH UPDATE: Client reports no mental issues this week.
HOUSING UPDATE: Client is 2010E and housing package completed and circulating. Client report she is actively searching for an apartment with Spring Creek and she is on a waiting list.
CM mentioned to the client due to her uncontrolled seizure disorder she is not a candidate to live on her own. Client report she will not accept an SRO.

REVIEW ILP AND CLIENT RIGHTS AND CLIENT CODE OF CONDUCT.
CM reiterates the shelter rule and regulation and client must adhere to the shelter rule and regulation and 10pm curfew.

WORKER ASSESSMENT: Plan of action: Client must participate in a substance & mental health program to address her drug dependency and major depression; client must continue with medication regimen. Client is not a candidate to live independently and her housing goal is unrealistic. Client must be open to a 24 hour SRO with case management on site. CM reviewed Bi-Weekly ILP. Client agreed and signed.

    At this point in the conversation Pamela raised her voice and began to discuss issues that she felt she needed assistance with. Pamela indicated that she felt a lack of assistance from SC so SC began to describe the role/responsibility of PCA vs. informal support. Despite SC attempting to diffuse Pamela’s strong feelings, Pamela continued to raise her voice and express great frustration.
