Sub Acute Unit Reflective Report
During the time of my career working as an LPN, I decided to work on a Sub-Acute unit in a nursing home. The unit consisted of 30 beds and each nurse assigned 15 patients. One night during the evening shift, I noticed the other nurse administering medications in the hallway to a patient in a wheelchair. I approached the nurse and questioned if it is appropriate to dispense medications in the hallway related to the policy of the company. I further elaborated on during orientation, the preceptor expressed the importance maintaining a patient privacy and dignity and questioned if this was against policy.     The nurse delegated tasks to aides which was expected. Two patients asked the nurse if she could help them change their brief and the nurse was busy in a med pass and told the patient that she would get their aid. We spent the first hour of the shift going over the diabetic patients and finding out what their sugar levels were to be able to properly prioritize the most critical patient before dinner. The LPN that I shadowed did not fill the expectation of cleanliness however.     Mr. E is a 67-year-old male who has been brought to the emergency department for lung congestion. Mr. E is diagnosed with pneumonia secondary to aspiration and Dr. K has determined that the best course of action is to intubate Mr. E. The nurse on duty discovers that Mr. E has an advanced directive that clearly states he does not want to be intubated.     Mary (changed name) was admitted with right cerebrovascular accident (CVA) and was now just waiting for residential placement. One of the side effects left over from her stroke was that she had a drop-foot for which she had a splint and a walking stick; she had declined to use a frame.     If I was faces with this situation I would first talk to the nurses who were humiliating the patients. While confronting the nurses I would make sure that they understand why patients especially the elderly should be treated with respect. By using the tradition approach, I can appeal to their beliefs about how people should be cared for. This approach may not be effective if the nurses to not see the situation the way everyone else does. The authority approach would be more effective in this situation.     PROCEDURE: The patient was brought into the operating room and placed on the operating table where spinal anesthesia was induced. The right lower extremity was prepped with bednine solution. A newmatic ankle tourniquet was placed over Webril padding above the right ankle joint. The leg was evaluated for proximately two minutes and the tourniquet was raised to 275 millimeters of mercury. An S-shaped curved incision through the right plantar fore foot ulceration was made down to abyss cavity where a moderate amount of sanguinopurulent discharge was noted. There was minimal odor noted to the region. There was mild venus bleeding noted on the edges of the incision. The cavity was probed and found to go deep into the musculatory layer plain between the bone and fascia.
