Corp. 6751 Ayala Avenue, Makati City, do hereby appoint, name and constitute SALVE BERNAS, of legal age, Filipino, SINGLE, __________________________; my true and lawful for me and my name place and stead, and for my use and benefit, to do and performed the following acts and things, to wit;
1. To submit and process my application of business sole proprietor at the Department of Trade and Industry.
2. To submit and process my application of registration of my business at the Bureau of Internal Revenue.
3. To submit and process all pertinent documents for Business Permit at the Makati and Taguig City.
4. To pay out or discharge or withdraw any funds belonging to me for the renovations of my business.
5. In general, to have full and complete management of my business ands properties in Metro Manila every kind whatsoever and whenever situated and to do any act or thing in relation thereto which may deem advisable.
GIVING AND GRANTING unto our representative full power and authority to execute and perform every act necessary and required in connection with these presents and hereby ratifying and confirming all that SALVE BERNAS may do by virtue of these presents.
