CM normally meet with the client every Tuesday for face to face meetings. Client was no show. CM inquires the reason client was no show on 2/16/2016. Client reported she was not available because she had family emergency. On 2/17/2016, CM met with the client and completed Bi-Weekly ILP Review.
In the meeting client was dressed properly and she was well groomed. In the meeting client was upset. CM inquires the reason client was upset. Client reported she observed many clients are viewing housing in the Bronx and she is not getting invitation to tour apartment in the Bronx. Client reported she feels discriminated that she is not given any appointment to tour housing. CM explained to the client every resident in the shelter situation is different and some client doesn’t have any portion to their Linc

MEDICAL UPDATE: Client continues to report arthritis in her left leg and hand, high blood pressure, stomach hernia and hip replacement. She appear to be upset when she reported no upcoming doctor’s appointment.

She continues to report no changes in her medications.

MENTAL HEALTH UPDATE: Client reported she is currently participating in mental health counseling at After Hours Project. Ms. Christina Coon/Counselor tel# 718-249-0755. Client continues to report she is enjoyed every individual session with her counselor; has she is building coping skills. She also reported no medication was prescribed. SUBSTANCE ABUSE UPDATE: Client has a history of alcohol abuse and she reported she drinks two beers a day, and if she need help she will seek help.

HOUSING UPDATE: Client has a Section 8 voucher which states it was extended until March.. Client also has a Linc IV voucher. Client submit apartment search log as proof of apartment search.
She also submitted an Application for River Park Residential Apartment. Client reported she is housing ready and she is waiting for DHS Manifest to view
