LEG:
Client discussed upcoming court appointment (scheduled for 2/3/16) and the anxiety attached to this event. Client expressed being worried about the impact of past absenteeism from PCO. Fore-mentioned topic encompassed the repercussions absences will have on his mandate being lifted. Client and counselor subsequently discussed in detail the minimal expectations of attendance while enrolled in PCO.

CHEM/DEP:
Client reported that he has abstained from all drug use and has no desire to use.

FAM:
Client explained how his recent absences from both group and individual treatment where a result of his mothers back injury and having to take over responsibility for baby sitting his niece. Client reported that his mother's health has been approving

    History of Chief Complaint: Ms. K has reported having mood swings at an early age. She has attempted suicide at the age of 14 by ingesting codeine pills.
