Child and mother arrived at the hospital at approximately 5:00 a.m. to register for outpatient myringotomy surgery. Mother registered 3 year old child and was escorted to pre-op surgery area for further processing. Mother signed consents; child was gowned and assessed for surgery, i.v. and pre-op medication given. Mother informed pre-op nurse that she would be leaving while child was in the OR and would be returning once child was out of recovery. Pre-op nurse gave the mother an estimated time of 1 hour and 45 minutes for surgery and recovery time. Mother gave the nurse her cell phone number with instructions to call her if child got out of surgery earlier. Pre-op nurse recorded phone numbers and instructions in her personal notebook.
Child taken to OR at approximately 5:45 and was sent to recovery at approximately 6:30. Unclear if OR nurse tried to contact mother when child came out of surgery, also unclear if surgeon tried to contact mother about surgery outcomes. Child was in recovery for approximately 1 hour. Recovery nurse paged mother with no response. Recovery nurse also went to waiting room looking for mother. Recovery nurse did not retrace child’s path for further information.
Child moved to the discharge nurse at approximately 7:30 a.m. Mother paged with no response. Discharge nurse did not retrace child’s path for further information. Security called discharge area to ask if child’s father could come back. Discharge nurse said yes and noted that the child was very happy and relieved to see father. Discharge nurse waited until 8:00 before releasing child into fathers care. Unclear if this discharge was recorded in patients chart.
Mother arrived at 8:35 and was informed that the child had been released. Discharge nurse was not able to give accounting of where or with whom the child had been released and security was called at 9:00 and the hospital was put on a ‘code pink’. Local law enforcement was also notified.

    The obvious problem is lack of communication, which was pointed out by nearly every employee involved in the sentinel event. The most effective way of making sure this does not happen is to create a risk management plan that begins the minute the patient walks in the door. NCH should create a protocol where the registrar takes down all information, including anything regarding custody of the child and who is allowed to pick up the underage patient. A code can then be administered to each family and wristbands can be printed with that code, and if the person picking up the child at discharge does not have the same bracelet, then he or she is to remain in custody of the discharge nurse until the legal guardian of the child (or any adults that were predetermined by the dominant guardian to facilitate the pickup)…

Although, I do believe that the method of discharge planning at my facility is consistent with information I researched and explained. After evaluating the inpatient discharge teaching care plan that my nurse reviewed with my patient it was clear that there was attainable measurable goals made. These goals were achieved my effective nursing interventions such as having the patient use her incentive spirometer to prevent fluid and/or mucus from building up in her lungs, walking to increase circulation and decrease her chances of developing bed sores, and maintained free of infections by keeping incisions clean and dry. I believe that my discharge care related to my two chosen articles. It is similar to my article, Timeliness of Outpatient Follow-up: An Evidence-Based Approach for Planning After Hospital Discharge, because follow- ups are an important aspect to decrease the risk of readmission. Due my patient having multiple chronic conditions it is critical to assume that her chances of readmission is high. Therefore, it is important that my patient received a thorough and detailed discharge. Afterwards, it is important that she receive a timely follow- up within the next seven days to confirm compliance and assist in any additional questions. Therefore, it would be in my best interest to call my patient again because she did not answer to review her discharge care plan. My discharge care related to my article, On the threshold: older people’s concerns about needs after discharge from hospital, because addressing any patient’s concerns is pertinent.
