Since the test in an integrated one, it will be divided into PARTS instead of using headlines as before (READING, WRITING, etc).
PART 1
Read the following text and then answer the questions below (2.5 Points):
The text will be on a topic discussed in one of the units the quiz covers. It will include some vocabulary discussed in the units and others which students will have to guess their meaning from the text (a strategy students must be taught). The text will be written (or adapted) in a way that we have to make sure it includes some grammatical rules students have taken in the units. This part could include 5 questions each worth 1/2 point.

PART 2
Here (just as a thought) we can have a listening part. Here students will be required to use two of their skills like listening and writing and their vocabulary knowledge. Students will have to write their answers after careful listening to the audio or even video!
(BUT, for quiz 1 this could be problematic if we ask students to write their answers in full sentences. This is because in the first three units, students are simply introduced the Capital Letters' and contractions' writing skills. Thus, students' answers will be written in words (vocabulary knowledge).
PART 3
We could also have some integrated fill in the blank questions. See the sample below:
