Existing cases range from 1.8 to 7.6 every 100,000 cases a year in parts of the continental U.S. Incident rates in caucasians in Rochester tripled from 1.5/100,000 in the 1950-1979 cohorts to 5.6/100,000 in the 1980-1992 cohorts.
