Jane Doe called me on the phone one day while I was at the Community Center working at the help desk. We set up the initial interview for the following day. The importance of the initial interview is for the personal trainer and the client to assess compatibility, develop goals, and establish a client-trainer agreement. When Jane first arrives for the interview I will provide a description of the services available and that I can provide her. I will explain to her that I am certified through the National Strength and Conditioning Association as of May 2012 and I have completed an Essentials of Personal Training class for credit at Northern Michigan University. I will explain to her the mission statement of the Community Center (mission statement of NSCA is to unify members and facilitate a professional exchange of ideas in strength development as it relates to the improvements of athletic performance and fitness) unique features to the program, and where and when services are available. I will then need to evaluate her readiness for fitness by assessing her level of motivation and commitment. I will ask her to discuss her past experience with exercise, time management skills, and possible obstacles that would prevent her from meeting with me. I would have her complete an attitudinal…

When my clinical coach approached her asking if it was ok to sit and chat with her once the break was completed. She agreed to talk in her room as we sat on her bed. GOAL: To have Ms. A engage in a therapeutic relationship. I’m hoping to have open dialogue on her experiences on a lock down ward. SUMMARY: 1. Was the goal reached? Explain if the goal was met or not met and include rationale for your explanation. The goal was partially met. I had interacted with the patient briefly on a number of occasions in the dining room through small talk prior to our talk. The patient was more apprehensive to keep the conversation going and withdrew due to fatigue. 2. Which phases of a relationship were demonstrated? The orientation phase was completed. Ms. A abruptly ended the conversation during the working phase of therapeutic communication. The subject matter became very close and personal on subject matter to which she presented in the emergency unit. 3. In order to gain Mary's trust and reliability, as a manager of the care home I have to advice Mary's social worker to offer her the affection and support she has never received, in the care home. In order to effectively inform the social worker about the details of Mary's situation- a very thorough verbal discussion will have to take place. This also involves a group conversation with the social worker and Mary, who can assist her point of views- as well as her troubles being expressed. This way, we would be able to understand Mary's personal perspective as well as being able to find the solution to gain her trust in the care home. On the other hand, Mary's slurred speech and hearing impairment may be an issue to deal with in terms of maintaining a conversation, however there are many ways to effectively overcome these struggles and help her enrich trustworthiness in the care home. It is important to create a line of communication between myself, the social worker and the patient so that we can already build a stable relationship that has been gained through communication, involving the exchange of intentions and thoughts which ensues the gaining of sympathy and mutual understanding. The aim of this interaction is to give Mary the love and care she has not received in her abusive home, as…
