Perhaps fear of changing themselves or feat that by changing themselves it may change those around them. From personal experience I have been with a counselor for an initial visit and chose not to return. For myself, the reason was not a good fit, and some unethical statements he made towards me. And in marriage counseling if both parties are not making an effort then it can not work.
