    The overall goal of the grant proposal is to get funding support for a patient accessible portal which allows diabetic patients to become proactive in their health and wellness to enhance their quality of life. The portal will have various features such as interactions with physicians, ability to record health vitals, monitor health status, follow recommended lifestyle changes to improve quality of life, and alert patients before serious conditions may arise.
