Primary Care Executive Summary
While in the facility Mr. Smith develops a slight cough, the physician is able to order a mobile chest x-ray to ensure that Mr. Smith's cough is not developing into pneumonia. The provider team is able to prevent Mr. Smith from having to go to the emergency room for his cough that was not related to pneumonia, and treated him in the facility. Upon discharge from the skilled nursing facility it is determined that Mr. Smith is in need of additional support for his nutrition and has not seen his primary care doctor in over three years. The social worker assigned to the facility Mr. Smith assists in connecting him to a meal program within the hospital network geared towards healthy meal options for seniors and connects to a new primary care physician within the hospital network, as his previously has retired. The team was able to ensure Mr. Smith was able to receive all of his care in the facility and got connected to resources after his discharge, many of which were part of the hospital network services.
The multiple levels of leadership provide support for day-to-day operations, as well as the directional growth of the department. The three leadership practices of the primary leader for the success of the program are organizational commitment,

It is essential to the program for the leader to collaborate with others within the hospital network and especially outside the hospital, most specifically with the Skilled Nursing Facilities. Collaboration internally assures the patients, as well as staff, have access to additional resources within the hospital network. External collaborations assure the providers have facilities with patients to see and provide care to. Collaborations must be done strategically to meet the objectives of the network efficiently. It is not efficient for the providers to work in facilities where there is a low volume of patients or the physical distance is too

    Inter-professional collaborations are invaluable relationships between healthcare professionals that are formed with the sole purpose of providing optimum help for a service user (Hornby and Atkins, 2000). Goodman and Clemow (2010) explain that collaborative practice is actually based around two ideas. The first is that nurses need to be able to work with other professionals, and the second is that they need to be able to work with ‘people’. They go on to explain that the concept of collaborative practice is based on the idea that excellent patient care relies on the expertise of a number of care providers. Day (2006) points out that actually defining what collaborative working is, is very complex, and that in order to understand what exactly collaborative working means within the context of nursing, an understanding of what collaboration is, is required, along with the ability to apply this understanding to a number of other terms which are often used both interchangeably and synonymously with this one in literature. According to Day, collaboration can be defined as individuals or agencies working together in order to achieve something that neither can achieve alone.
