Patient Case Summary
DOI: 10/22/2012. Patient is a 50-year-old female certified nursing assistant who sustained a work-related injury to her back while she was lifting a patient to bathe. As per OMNI entry, the patient is status post anterior cervical decompression and fusion in 2013, and laminectomy/partial facetectomy at C3-7 and segmental instrumentation fusion on 7/29/2013.
Per medical report dated 4/26/16, patient’s medications include Percocet, Flexeril, and Prozac. Examination of the lumbar spine revealed tenderness. Straight leg raise test on the left causes posterior thigh pain.
Based on the medical report dated 07/14/16, the patient presents with a history of pain and severe spasm in the lumbar region that occurs constantly. The patient has numbness

    He does have some pain control with the use of the Duragesic patches and Norco. It was noted that the psychological evaluation has helped him cope with pain level. The patient is complaining of a new pain in lower back, that is worse when he sits and it is radiating down the legs. He is wondering if there is anything that can be done.     IW reports constant neck pain as aching with exacerbations and radiation to bilateral shoulders/upper extremities. IW notes intermittent numbness and tingling to bilateral hands/fingers.
