Government’s approach to dealing with New Zealand’s childhood obesity epidemic.

I am sure many of you found recent comments in the UK Chief Medical Officer’s annual report as chilling as I did.

He said the reality of the obesity epidemic would mean many parents would outlive their children.

We do not want that to happen here. We are facing a child obesity epidemic in New Zealand, but we can still do something about it.

That is why it is so important to be part of a positive symposium like this that aims to “be part of the solution”, to stop laying blame, and to plan the way forward.

I think it is great to get in on the ground floor, to plan for the future, not the let the future hit is. I want to thank Professor Ian Watson, the Principal of Massey University’s Albany Campus, the Symposium Coordinator Rose Carr, and all the other leading health, food, industry and media experts who have willingly become involved in the spirit of this Symposium.

We will know a lot more detail about the extent of childhood obesity in New Zealand when the Children’s Nutrition Survey is published late this year.

This will be our first national survey of children, and includes information from over 3200 children aged between 5 and 14, randomly selected from schools around New Zealand.

But, before the survey is even completed, we already know a number of relevant facts.

The first of these is that obese children are more likely to become obese adults, thus increasing the risk of associated diseases, especially type 2 diabetes, of which New Zealand has an epidemic, heart disease, hypertension, some forms of cancer, and joint and skeletal problems.

We also know that one in seven New Zealand children suffer from obesity, and that this percentage is increasing.

The ‘Nutrition and the Burden of Disease’ report, which I launched last month, provides disturbing evidence about the destructive impact of poor nutrition.

The report revealed that two out of five deaths in New Zealand, or 11,000 deaths a year, are attributable to nutrition-related factors, with 8,000 to 9,000 related to diet and 2,000 to 3,000 related to physical inactivity.

The report showed the potential benefits to the health of New Zealanders from even modest improvements in our diet are considerable. Such modest improvements in diet could achieve health gains very quickly, preventing hundreds of deaths annually within just a few years.

Acknowledging the facts is one thing, but doing something about it is another.

We cannot change everyone’s attitudes to nutrition overnight, or over a week, a month or a year, but that is no excuse for not trying to create real awareness among New Zealanders.

I am a strong believer in education and encouragement as the best approach, although I know there are others who have a personal belief in a more regulatory approach.

Education and encouragement are certainly the guiding principals behind the ‘Healthy Eating: Healthy Action’ strategy I launched in March at Saint Anne’s school in Wellington. The strategy was developed over two years after extensive consultation, no doubt with many of you here today.

Healthy Eating: Healthy Action focuses on changing the environment of people to help them make healthy choices, and children and their families are a priority area within the strategy.

The strategy doesn't just focus on changing personal behaviors. It signals the need for major changes to reduce various social, cultural, economic and physical environmental barriers to eating well, being physically active and achieving a healthy weight.

Everyone needs to be involved in the solutions. The strategy calls on a range of sectors, such as transport, local government, health, education, and food, fitness and weight-loss industries to work together. That is why it is good to see so many sectors represented at this symposium today.

St Anne’s School and Beachhaven Primary School are both Health Promoting Schools, and offer a good example of the way various elements and agencies can be linked.

St Anne’s, for example, promotes the Walking School Bus, holds daily aerobic sessions, has a water-only and healthy lunch order policy, and links home and school together by inviting parents to the school to discuss issues like nutrition.

It is my hope that more and more schools will follow the example St Anne’s and Beachhaven provide.

The next step in terms of the Healthy Eating: Healthy Action strategy is to develop an action plan to accompany it, and this is happening now. This plan will focus on tackling health inequalities, and will encourage health providers to work with the food and fitness industries, local government and schools to provide coordinated nutrition and physical activity services.

The direct costs of obesity to the health sector were estimated in 2000/01 at $247.1million annually, or 2.5 percent of health spending that year.

But, of more importance are the costs of obesity to individuals, to families and to New Zealand as a whole.

Prevention is always better than cure, and prevention is easier too, provided you are committed to it. Creating environments for our children that encourage good nutrition and increased physical activity is a great start.

New Zealand’s approach to improving child health is taking place within an overall strategic framework, through both the New Zealand Health Strategy and the Primary Health Care Strategy as well as Healthy Eating: Healthy Action.

The basis of our primary health care approach is to provide low cost and accessible primary health care to all New Zealanders over the next eight to ten years through the development of Primary Health Organisations.

As you know, we are first of all targeting those living in low socio-economic, high health-need regions, and children and young people. I am delighted PHOs are already reaching more than twice as many people as we expected them to after one year of operation.

As we all look today to find a way forward to tackle child obesity in New Zealand, I urge you all to remember that if we really want to create a healthier environment for our young people, we need to listen to them and understand their needs.

Societal changes over the last 20 years have helped to create the current obesity epidemic.

While we cannot turn back the clock, we can draw on our experiences of when we were children and explore new innovative ways to make a difference, and we can listen to today’s young people about their experiences as well.

We hear a great deal these days, not only about the rising tide of obesity in children, but also the role of fast food advertising, television watching and many similar factors. Presumably because it is less politically respectable, we hear much less about parental factors.

First, of course, is the question of genetics. It seems probable there are genetic factors which, in the presence of an abundant food supply, make some people more likely to become obese than others. But the family environment must play a major role.

A new research paper from Brisbane throws light on this. The study has been following 2934 children from pregnancy onwards, and looked at their weight status at five and at 14 years. Some children were of normal body mass index (BMI) at both ages, some were overweight at 14 but not at five, some the reverse, and some were overweight at both ages. This was compared with the parents' status.

In the study, 68.8% were of normal weight at both ages; 14.5% were normal at five years but overweight/obese at 14 years; 6.0% were overweight/obese at five but not at 14 years (mostly girls); and 10.8% were too large at both ages.

Children whose parents were overweight/obese were over six times more likely to move from normal weight at five to overweight at 14 years, and were over seven times more likely to be overweight at both ages. If only one parent was overweight, it was twice as likely to be the mother, although overweight women are more likely to be parents with overweight men so the father's fatness was also strongly related. Factors not associated with the child's leanness or overweightness included family income, mother's education, race or the child's position in the family with regard to siblings.

This study provides further strong support to suggest the family environment, probably both dietary habits and exercise customs, is a primary factor in the child obesity epidemic. Interventions involving the parents must be employed to combat the problem.

In developed nations all over the world – particularly the United Kingdom and the United States – problems with obesity have become such a major issue that they are discussed widely on television, on the Internet, on the radio, and in everyday conversation. In addition to bad eating habits, one of the main problems that causes childhood obesity is societal and relates to the lack of physical activity in our day to day lives. As the world becomes more complex, technology continues to make advances that encourage us to sit around all day doing little to nothing physically. Why bother when we have video games, computers, and televisions to entertain us? Why bother walking anywhere or riding a bike when we can simply get in a car or take public transportation? What it comes down to is a simple lack of movement in our day to day lives. Not only does physical inaction cause overweight and obesity, it can also contribute to a variety of health factors that hit people later in life, including type 2 diabetes, heart disease, and stroke. In England alone, the estimated costs to the public of illnesses that can be traced back to inactivity amount to over 8.2 billion pounds! The fact is, being active is no longer a simple option. If we want to live long and fulfilling lives, we must begin to become more physically active. One of the sad facts of our time is that young people have fewer and fewer chances to be physically active. In elementary and primary schools, quite often there is a lack of space and equipment for serious physical education, not to mention fewer and fewer teachers with specialized training in this area. Children spent the vast majority of their time at school sitting down behind a desk. The vast majority of six to eight year old in the United Kingdom are allowed only thirty minutes of regulated physical education in their weekly school curriculums! The game fields and playgrounds are being sold off or abandoned. At home, children are discouraged from playing outside due to their parents’ concerns with safety. Here’s one idea: Instead of blaming technology for all our problems, is there a way that technology can be utilized to help children become more physically active? Some video games are encouraging just that. While video games have a bad reputation for causing children to lead sedentary existences, some new games have encouraged physical activity, integrating such acts as golf swings, dance, and martial arts movements in to their programming. This new form of combining fun with technological advancement has been christened “extertainment,” also known as “exergaming.” Some schools in the United States have even adapted this form of exercise as a substitute for activities such as baseball. While exertainment has of course been controversial to some extents, it has proven to be largely successful, in that students often choose to use the equipment at lunch time and after school – typically times of relaxation when students are sedentary.

Physical inactivity and obesity contribute enormously to the current burden of disease in Australia, as in all Western Countries. Although diet and exercise have been linked to health since antiquity, the connection has recently experienced a revival of interest. There is mounting evidence that the current prevalence of obesity is more closely related to decreases in energy expenditure than to the traditionally accepted imbalance between consumption and expenditure, and that physical activity is therefore crucial in maintaining weight loss. With overweight[1] and obesity[2] now the norm in Australia, physical inactivity is a major issue within and beyond the health sector, and should be a concern of all health practitioners. This paper provides an overview of the relationship between obesity and physical inactivity and reflects on some strategies for increasing physical activity in the obese. More research on effective strategies to promote physical activity in all its forms (eg. exercise, transportation and incidental activity) is needed.

First of all, find out if the child has any hereditary issues that are contributing to their weight. Then, make sure they are getting the right kind of diet in the right quantity and that they are performing regular exercise.

Make sure that you are limiting the amount of time that the game consoles and television are on, and spend plenty of time playing outdoors with your kids. You can take family walks or bike rides, or organize a neighborhood basketball game. Keep your kids active as much as possible, and another cause of childhood obesity will fall by the wayside. The budget for an individual school could be greatly impacted by these tests scores. The school scores on the standardized tests can affect even the local real estate prices. It is also important to get regular physical activity if you want to maintain a healthy weight. Encourage your kids to play outdoor games like cricket, basketball, football etc. Discourage them from playing computer games, television etc. There should be timetable for children to watch television and computer games. They should be made compulsory to go out and play outdoor games. Also they should do regular exercise and keep themselves active and fit. Parents should keep a watch on their children to make sure all these things are strictly followed. If it is not strictly followed, then it can affect the physical status of the child and make him more lazy and inactive. Physical education needs improvement

These unhealthy alternatives include pizza, popcorn, burgers which are tasty but low on nutrition value. Children have a tendency to get attracted to such junk food and then neglect food with good nutritional value. Hence children should be encouraged to go for fruits and vegetables instead of junk food like burger, pizza, popcorn etc. Also one major problem is that there are many snack vending machines in schools. So students can always avail of these vending machines and there cannot be any check on this snack food, which they eat. The children who need physical education so much now get none. Many of the school districts have cut physical education out of the school curriculum almost completely. The child obesity solution requires physical activity for these children. The parents and schools should work together for this as well. Regular exercise is another important factor in reversing the numbers in the current child obesity graphs. Recess time and play time after lunch has been cut considerably because the schools feel the pressure to increase the averages on the standardized tests. If it is not strictly followed, then it can affect the physical status of the child and make him more lazy and inactive.

After playing these outdoor games and doing regular exercise, children can burn out their extra calories and remain fit and healthy. When it comes to child obesity, doctors have plenty of healthy recommendations for parents that will ensure that their kids stay slim and trim. But parents should make sure that they lead by example instead of forcing their kids to do regular exercise. If parents have good eating habits kids will follow them also.
