1. Expound on how you provide a safe environment for delivery of nursing care.

I provided a safe environment for my patient who was about 80 year’s old suffering from Alzheimer’s disease. My patient was only alert and oriented to person. So I made sure I did focused rounding on my patient was also on soft wrist restraints.

2. Give details of an instance when you demonstrated competency in skills performed in the clinical setting.

I showed competency on making sure I was checking my patient’s soft writs restraints every two hours. I made sure I took them off one at a time, and look for any skin break down and that the patient was maintaining good peripheral circulation in his wrists.

I communicated this to my instructor in an appropriate way. He help me gather oxygen equipment and we place the patient on two litters of oxygen. Fifteen minute later I rechecked his pulse oximeter and he was at 94%.

5. Elaborate on how you identify your self-learning needs

The learning needs I identified in my self was write all my assessment data down on paper. A lot of times I become busy and chart based on my memory of the patient. My instructor helped me to realize that doing this is not safe to rely on.

6. Explain an instance where you implement patient teaching on assigned patients, families and groups.

I implemented patient teaching when my patient had trouble breathing and was on a liquid restriction diet due to her congestive heart failure. I helped her to utilize pursed lipped breathing. I always taught her why it’s so important to limit her fluid because she was having a hard time understanding why she couldn’t just drink what she wanted.

    This act created a major revision of standards of care for nursing homes. This legislation also changed the expectations and the quality of care that patients should receive in long term care facilities. This Nursing Home Reform Act passed by congress specifically stated “that each residents have the right to be free from any physical or chemical restraints imposed for purposes of discipline or convenience and not required to treat the resident’s medical symptoms”. While there may be some benefits to using restraints in nursing homes, however, studies have shown that using restraints in nursing homes negatively impacts patients and for the most part does not prevent them from falling or from other incidents that may occur. There are very high levels of risks associated with the use of restraints (Lapane,150). The OBRA act has encouraged providers to use other alternatives and methods other than restraint because it has been proven to be quite harmful. There are various risked associated with restraints and of these risk include patients potentially patients even losing their lives. Because of the nature of the restraint on patients, patients sometimes end up losing muscle and bone density which then results to immobility and immobility itself results in loss of endurance and muscle strength (Lane 2008). When a patient has lost endurance and muscle strength, they have problems with balance and coordination which makes patients even more prone to falling (Capezuti &Wagner, 142). The use of restraints does not only affect patients physically but it also affects them psychologically. When a patient is restrained, they may experience depression. In addition to that, patients also get humiliated and feel as if they are not being treated with dignity.
