Personal Wellness Planning Assessment

Warm-Up (Questions to help you prepare to write your goals)
1. Describe your current level of physical activity. How often do you participate in moderate to high intensity activity in an average week?
A. On a scale to 1-10 I would give myself a 6 on how physically I am on a weekly level of activity participation. I usually go running in the morning. And in the afternoon I play basketball and go for a swim

2. Discuss at least three physical activities you enjoy participating in or wish to participate in
A. I enjoy participating in basketball football and baseball. They are a lot of fun and allow me to connect better with my friends

Goals
3. Create at least one goal for each of the following wellness categories. Personalize each of your goals for the time enrolled in the course. Be sure that each goal is measureable, attainable, and has a specific deadline. Also, be sure each goal is written in complete sentences. Physical Wellness Goal:
A. I want to be able to fun a mile without stopping at a constant speed. This will help me keep losing weight and also boost my endurance

Emotional Wellness Goal:
A. I will hangout with my family and grow my connection with them by playing family games and express how I am feeling and seeing how they are doing

Social Wellness Goal:
A. I want to hangout with my friends and express my views on life and tell them how I am doing in life. I also want to make new friends and grow socially

Academic Wellness Goal:
A.I want to maintain straight As and get into fsu. Applications start in a week and I hope and it is my goal to get in. fsu is my dream school Cool Down (Reflection questions based on the goals you’ve written)

    Most people will think of physical wellness as how they are feeling, whether they are sick, tired, or injured in some sort of way. According to Harper, he defines physical wellness as the ability to perform daily activities in a safe, capable, and efficient manner, as well as the physical capacity to respond effectively in unusual situations (2008). Qualities of a person that inhibits physical wellness, as discussed by the Mayo Foundation for Medical Education and Research are controlling your weight, combating health conditions and diseases, improves your mood, boosts your energy, sleep better, put the spark back into your love life, and physical fitness can be fun (2011). Furthermore the general goal is to aim for at least 30 minutes of physical activity every day in order to attain the above qualities (Mayo Foundation for Medical Education and Research, 2011).
