The provided scenario gives an account of a busy emergency department with competent staff, and the multiple errors that led up to the most severe error possible in healthcare, unnecessary death of a patient. A root cause analysis (RCA) can be utilized to help understand the systems at fault within the facility so that improvements can be determined and implemented to prevent any future occurrences (Cherry, 456). RCAs focus on systems rather than blaming individuals involved, therefore they are only appropriate in cases where there has been no willful negligence or criminal acts (Huber & Ogrinc, 2014). The overall purpose of RCAs is to find out the causes of the adverse event and determine how to keep similar errors from repeating in the future.…

Lets take a journey. A journey back to a time and a place that is unknown to us without the history and expression of Literature and Art. These moments are the expression of color, the fine detail, the heroics, and the stories that bring us to our current and most knowledgeable time in literature and the arts. Neoclassicism and Romanticism are two very important time periods in the literary movements in English literature that helped shape our way of life today. Distributive shock is the decrease in intravascular volume caused by dilation of the vessels and capillaries. It is important to note that there is no actual fluid loss or blood from the vessels but only lost of volume. This volume has become inadequate to fill the increased size and capacity of the vessels. In some cases they dilate so much that capillaries will actually become permeable, allowing fluid out of the capillaries. This leakage causes a reduction in the pre-load, which reduces stroke volume, cardiac output, and systolic blood pressure. Now there is three forms of distributive shock: septic, Neurogenic, and Anaphylactic shock.…

When people think toxic shock syndrome they often automatically think women and tampons, but in reality it can affect men, children, and postmenopausal women. Toxic shock syndrome is rare, but it is also life threatening. It often results from complications of types of bacterial infections such as toxins from Staphylococcus aureus or group A streptococcus bacteria. Toxic shock syndrome is associated with having viral infections, having cuts or burns on the skin, having recent surgery, using contraceptive sponges, diaphragms, or superabsorbent tampons. Toxic shock syndrome can progress rapidly, and therefore should not be taken lightly. Complications from it include shock, renal failure, and death. There is not a test for toxic shock syndrome, but blood or urine samples may be taken to check for the presence of staph or strep. Also the swabbing of the vagina, cervix, or throat may occur for laboratory analysis. A doctor may also order a CT scan, chest X-ray, or lumbar puncture to see the extent of the illness since toxic shock syndrome affect multiple organs. It is likely a person will be hospitalized if they develop…
