2013 Transitional Support Scholarship

The Solano County First Place for Youth Independent Living Skills Program is accepting applications for the 2013 “Transitional Support” Scholarship. This is an opportunity for youth to apply to receive financial support, support with education and/ or household items for independent living. PLEASE INDICATE THE TYPE OF SCHOLARSHIP YOU ARE REQUESTING

 Educational: financial support with post secondary education/ school supplies  Transitional/ Independent Living: financial support with housing / household items APPLICANT ELIGIBILITY CRITERIA Applicant must meet the following requirements to be considered for a financial award and/or education / household supplies:  Eligible to receive ILSP services (Independent Living Skills Program)  Obtain High School Diploma or GED between July 1, 2012 to July 30, 2013 REQUIRED DOCUMENTATION: Applications received without all the required items below will not be considered for a scholarship award.  Completed Application Form  Proof of enrollment to post secondary educational institution (If Applicable)  Proof that High School Diploma or GED requirements completed by July 30, 2013 (Unofficial transcript or letter from school official)  Short Answer Essay  Personal Statement  Signed and Dated Release Forms  One Letter of Recommendation
