Ms. Sadia Case Summary
UCM: CPSW called Ms. Sadia to schedule a home visit and to discuss about her UA's and the updated therapy she had mentioned during the last home visit. Ms. Sadia stated that she has called the therapy and still waiting a phone call from her therapy to schedule an appointment. CPSW reminded Ms. Sadia that she needs to start a therapy asap and she needs to fallow her case plan. Ms. Sadia seemed to be reluctant and requires reminders about her case plan and what needs to be completed. She has not been cooperative with the department's goal and belive that the case is opened because of her case and she has done nothing to children. CPSW did a therapy referral for Ms. Sadia even though Ms. Sadia stating that she will be scheduling a therapy since
