Monitoring: The purpose of monitoring is to ensure the progress of an assessment, planning and implementation checking that the planned objectives are being achieved. It is also to help with improving services. This process is usually ongoing and regular depending on the complexity of the case.

    I recognise this assessment is ongoing and involved weekly observations to identify progress and monitor the services.
