On 12/17/2015 client Wanda Hernandez was case conference with CMS Pimentel and CM Watersoon because client has failed to attend ILP meeting with CM. After, Case Conference HS De La Torre met with client in order to develop a Housing Plan.

Ms. Hernandez is currently residing at 220 East 197 St. Unit 2I Bronx, NY admission date 07/22/13. Family consist of Wanda Hernandez (self 30y); Steven Jaquez (son 10y); Yoel Ventura (son 7y); Sophia Hernandez (daughter 1y); Noemely Ventura (daughter 9y). Client last apartment was located at 1971 Marmion Ave. Bronx, NY. Client was evicted from this location. Client did not provide any reason of losing the apartment. Ms. Hernandez informed the HS that no one will move in with them. Also, anybody in the family
