Dear Regional Vice-President Cardona,
In accordance with Sanggunian Resolution No. ________, the Municipality of_____________ hereby declare its commitment or continued commitment to participate under the National Health Insurance Program (NHIP) as a sponsored and to undertake the following necessary steps to ensure successful implementation of the said Program, as prescribed in the National Health Insurance Act of 2013 (Republic Act No. 10606):
1. To provide the needed premium contributions of all barangay health workers, the nutrition scholars and other barangay workers and volunteers in our locality;
2. To provide the needed premium contribution of an enrolled women who are about to give birth and of those critical poor patients identified in our provincial/ city hospitals through the Point of Care enrollment;
3. To provide the needed premium contribution, through a cost sharing mechanism, of families in the informal economy who did not qualify for a full subsidy by national government as determine by the Department of Social Welfare and Development (DSWD);
4. To assist PhilHealth in the conduct of the information, education and communication activities to our sponsored members, among others; and
5. In coordination with PhilHealth, to ensure that all our constituents are enrolled with the National Health Insurance Program.
Under this Program, we commit to pay the prescribe premium contribution (monthly, quarterly, semi-annually, annually) and to remit the same to PhilHealth within Thirty (30) calendar days in from receipt of the belling statement from the latter. Please find attached copy of the Sanggunian Resolution for your guidance and reference.
