A doula is a professional trained in childbirth who provides emotional, physical and informational support to the mother who is expecting, is experiencing labor, or has recently given birth. The doula’s purpose is to help women have a safe, memorable and empowering birthing experience because she recognizes birth as a key experience the mother will remember all her life. The doula understands the physiology of birth and the emotional needs of a woman in labor. She assists the woman in preparing for and carrying out her plans for birth. Stays with the woman throughout the labor and facilitates communication between the laboring woman, her partner and her clinical care providers. In 2014 there were 3,985,924 births in the U.S, an increase by 1% from last year. The fertility rate was 62.9 births per 1000 in women between the ages of 15-44 (NCHS site). According to current literature, non-Hispanic White women tend to have favorable reproductive outcomes while non-Hispanic Black women tend to be disproportionately affected by adverse reproductive outcomes, in addition to having the worst reproductive outcome among Hispanic, Asians and Whites (Bediako) (Jonelle). One study examined the role of race and socioeconomic status on reproductive outcome and found that non-Hispanic Black women had higher rates of preterm and low birth weight among each stratified socioeconomic class (Dang). Infant mortality is a worldwide indicator of a nation’s health status. Even though infant mortality has declined steadily over the past several decades, the United States ranks 24th in infant mortality, compared to other industrialized nations. Compared with the national average in 1996 of 7.2 deaths per 1,000 live births, the largest disparity was among blacks with a death rate of 14.2 per 1,000 live births in 1996, which is almost 2½ times that of white infants (6 deaths per 1,000 in 1996). American Indians as a whole had an infant death rate of 9 deaths per 1,000 live births in 1995, however some Indian communities have an infant mortality rate almost twice that of the national rate. The same applies to the Hispanic community (rate of 7.6 deaths per 1,000 births in 1995) and the Puerto Rican community (rate was 8.9 deaths per 1,000 births in 1995).…
