Outcome 1
1.1- Person centred values is about providing care to an individual to meet their needs. Just because two people might have the same medical condition, doesn't mean they require the same care and support. For example: treating people as individuals, supporting people to access their rights, give them the right of choice, assist them to be as independent as possible and treat them with dignity and respect.
1.2- We must take into account the individual history, needs, wishes and preferences when planning care and support. This involves asking the individual every aspect of their lives such as, what time they like to get up and have their meals, how they like their personal hygiene, what they like to wear, what they like to do during the day and also their religion

Arrange for them to speak to the care manager and encourage them to say how they feel and what they want. If they want to make a complaint then use the complaints procedure.

Outcome 6
6.1- A person identity makes them different from everyone else. They may have specific traits or interests and if these are repressed a person may feel withdrawn or unable to express themselves. Similar for self-image, if a person is not supported or if somehow "not allowed" to dress, style their hair, etc.. in the way they choose or if they are not supported properly to look presentable in a way they feel good, clean and tidy, this might affect they self-esteem.
6.2 - By letting the person express themselves about their likes and dislikes, their interests, their choices and supporting them in a way they feel good, clean and tidy, all these will contribute to their overall wellbeing and this will affect if they generally feel happy and good about themselves on a day to day basis.

7.1 - We use risk assessment for everything in our lives as well as when looking after of vulnerable
