How To Write A Reflective Account
An event occurred that did not exemplify best practice as a nursing student, and soon to be registered nurse was when I was administering medication last week on a respiratory floor. I had given my patient the medication, and realized there was no water at the bedside. I quickly went to get my patient water for her to swallow the capsules more easily from the bathroom. When I arrived back, she stated that she had already taken them. I had informed my preceptor and she advised me to look on her bed or the floor to see if she had discarded them there. We had looked around the proximity of the patient, and found two of the pills amongst her bed sheets. Elaborate
My objective recall to the situation was that I prepared my patient’s evening medications, and had given them to her. I had explained what each medication was and its purpose. She had agreed to take them. I had noticed that there was not water at the bedside, therefore I had went into the bathroom for a very short amount of time to fill up her cup with water for her to swallow the medication. When I came back and handed her the cup she stated “it is okay I already took them”. I did not observe the patient swallowing her medications, which were stool softeners.

Factors that have influenced this event is that the patient was a respiratory, and psychiatric patient. She had a history of severe depression, anorexia, seizures, and schizophrenia. The patient was on a form 1, then placed off of it on my shift as she left the hospital and was found at home without official discharge. Also, she had stated she had a bowel movement the night before and did not want the laxatives and stool softeners. She felt that she did not need the medication as she was not constipated.     The first alternative for the pharmacist was to get a thorough medical history of patient because of the medication that he was dispensing. When learning the various drugs and the combination of mixtures that should not have been made it would have been a red flag for him to spot.     On my recent placement in Hairmyres hospital I had been asked by a nurse if I wanted to do the medicine round with them to get an idea of what it was like, I had agreed to this and found it interesting, during the round the nurse showing me was asked by another member of staff to do something, the nurse then asked me if I would finish putting two more tablets in the container and giving it to the patient, having read over the NMC code of conduct and remembering this statement from it “As a professional, you are personally accountable for actions and omissions in your practice and must always be able to justify your decisions”.     I started my shift at 7:15 am. My work clothes are comfortable and unrestrictive; my footwear is flat, sensible and offers protection to my feet and ankles. When I arrive to the service user’s home, I knock on the door and wait to be let in either by one of the service user’s or by a work colleague. Once inside I greet the service user, if they are awake. I then head to the sleep in room, to start verbal handover with my work colleague. After the 15 minute handover, I then check and sign the handover sheet. Check the on call rota, should there be an incident or or accident. Also check the communication book and also check any money and medication, then checking the activity planner. Once the service user wakes, we say hello to each other, and chat for a short period. I encourage them to start their personal care. After, I ask them if they need any support with getting dressed and preparing breakast. The time is now 8:00 so I administrer the morning medication. I always wash my hands before handling food or medication and after assisting with any personal care. After the service user has had their breakfast. I support them with the washing up. I will then encourage the service user to clean their property. I will then support the indiviual in safely cleaning, all ares with correct products. After this I consult with the service user, about their activity plan for the day, if the service user decided to go into town.     I work in a Jewish care home on EMI dementia nursing unit for the elderly. My clients suffer from dementia. They are in different stages of that illness, from mild to severe. Some of them are in end of live care. My clients come from different backgrounds then me and where raised in a very different way to how I was raised. It is important that I will remember this and respect their beliefs, as we all have personal beliefs and preferences based on our background and upbringing. I have been brought up in a catholic society where religious education was compulsory, however we still studied various other religions and were taught to respect all people and there beliefs. Prayers were a daily thing at the end of each assembly and whether or not I wanted to pay attention to the prayer time I had to respect the people that did. Religion may affect people in many different ways, mainly because we all come from different backgrounds and are taught differently. However, it is important to respect how people honor their religion. I am an catholic, however I still make time to talk to my clients about their religion and sometimes have prayer time with them, and when possible I take the clients to synagogue. I spend my time with them on Saturdays (in Jewish culture Sabbath is a day of rest) even though my own beliefs teach me something different, I know it makes the client happy so it makes me happy to do this. I know it is something that they feel so strongly about and it is amazing at how much I really now look forward to going to synagogue with them.     One day I was ask to transfer Mrs X from bed into wheelchair. Mrs X is paralised so to transfer her a full body hoist is needed. I had to call for help, it wouldn’t be safe to do this by myself. So while I was waiting for the other carer to arrive I have explained to Mrs X what and how we are going to do it. I have also cheked the hoist and battery if its fully charged and operational. I put right size sling on Mrs X with her cooperation. I carefully manovered hoist close to bed, then I hooked sling onto the hoist with short hooks on to of the body and long hooks on bottom. That was when my collegue arrived to help me with transfer.We asked Mrs X if she is ready when she said yes by pushing button on hoist remote I raised Mrs X up, while my collegue was holding her to stop swinging. When she was up I reversed away from bed. When I was in positionfor lowering Mrs X I have opened legs of the hoist. When my collegue manouvered wheelchair into position I was supporting Mrs X on back and knees. When wheelchair was in position my collegue took over of supporting Mrs X and I asked both Mrs X and my collegue if they are ready to lower Mrs X into a wheelchair.
