Enclosed are two consent forms, a baseline questionnaire, and a baseline locator, and a postage-paid envelope in which to return these forms. Please read the entire consent from carefully nefore signing it. Keep one copy of the consent form for your records. If you have any questions about the form, please call Lynn or Vanda at 555-8706. Answer each question to the best of your ability. If there are some questions that you are not able to answer, indicate that on the form, and an interviewer will telephone you for more information.

You were justified in being angry. We are sorry for the problems you had with your Speedy Striders, Model X7, which forced you to drop out of Memorial Day Fun Run. I have telephoned your physical education instructor and sent her a letter explaining your reason for not completing the class assignment. We are grateful to you for bringing this matter to our attention. The photo you sent was especially helpful. After we received your letter, we conducted our investigation of our equipment. We discovered that the equipment use d in sewing the Model X7 shoe in faulty. All of our dealers now have been informed of this situation, and the model X7 shoes have been removed from the store shelves. Please take the damaged shoes to the Dequinder Mall store. The store manager, Mr. Kelly, has been contacted and instructed to refund your money in full.
