Outcome 3: Be able to minimise risk before moving and positioning individuals o If I identified any risks to the individual or myself before a procedure I would remove any obstacles, any faulty equipment and report to my manager and if the individual had any behavioural issues, health issues or refused assistance I would record in the care plan so everyone was aware of them and report to my manager so the correct treatment or assistance could be accessed. o Sometimes the individuals insight to their help required is not the same as stated in the care plan if I was to have this happen I would try to explain to the individual what is in their plan of care explaining the risks involved but if this did not resolve the issue I would contact my manager for guidance.
