HEENT Checklist

Eyes:

Identify anatomic landmarks of the eye. (Slide 2, lecture)
Palpebral fissure: opening between upper and lower lid; no lid lag, no widening; pull lids up/down, lower lid is at lower margin
Lacrimal apparatus: lacrimal puncta and gland (palpate), no swelling and pain.
Lids/Lashes: palpate/observe, lashes are everted
Conjunctiva: membrane covering the sclera and lids; pull upper/lower lids; look up down and side to side; no palnes
Sclera: white in color
Cornea: clear, transparent; have patient look up at light
Iris: color
Pupil(s): size

Assess visual acuity: light on bridge of nose; eyes should respond to light symmetrically.
Pupillary response: convergence; constricts towards eyes; dilates away from eyes
EOMs: 6 cardinal fields of gaze; no nystagmus present; eyes/lids move together
Corneal Light Reflex: constricts to light

Ears:

Identify anatomic landmarks of the ear. (Slide 28, lecture)
Landmarks: Helix, Antihelix, tragus, lobe, mastoid bone (look behind ear for lesions)
Identify the TMs (internal canal): no erythema, drainage, lesions, cerumen (external canal); TM there’s malleolus
Cone of light
Whisper test

Nose:
Identify anatomic landmarks of the nose. (Slide 35, lecture)
Symmetry of nose and nares. Test patency.
Look at nasal mucosa; red turbinates/moist; pink mucosa, no lesions/polyps, nasal turbinates. No nasal flaring

Throat/Mouth:
Identify the anatomic landmarks of the mouth and throat. (Slide 38, lecture)
Lips: palpate, no bleeding, cracks, no lesions, lips moisturized
Teeth and gums: teeth are in good condition; gums are moist, pink, and not bleeding.
Buccal mucosa: moist, pink, no bleeding.
Tongue: midline; no swelling, no lesions, no bleeding
Uvula: midline, goes up with phonation
Tonsils: tonsils are present, and not swollen.
