continuing life community.
Seniors can join the community
as active adults and live
independently knowing that if
and when the time comes where
they may need further
assistance of even skilled
nursing care they can remain in

comforting to know that you can
remain with friends close by,

GlenBrook is located adjacent to the La Costa
Glen continuing care retirement community,
they offer assisted living, skilled nursing care
as well as memory care for Dementia and
Alzheimer’s patients. GlenBrook is open to
the public as well as residents of La Costa
Glen residents. GlenBrook received and
overall five star rating from the federal
agency based on three critical areas, health
inspection results, quality measures and
staffing.

What is the role of the administration and case manager in this facility? How do their roles in this facility differ from the roles of administration and case manager in other facilities?
The directors and staff work to meet the
needs of the skilled nursing patients and
assisted living residents. GlenBrook has
case managers, RN’s, LVN’s, and
CNA’s(certified nursing assistants) for the
Dementia and Alchemizes Care.
GlenBrook does not have social workers
as other facilities do. GlenBrook’s staff
works together with the family to come up
with the best personal care plan for the
patient’s.
