G-CM: Case Management Case Study
G- CM met with client for a weekly case management, to continue assisting with his housing goals, and update his ISP goals.

I- CM inquired if client obtained his psychotropic medications from the VA. CM continued to assess client’s mental health and medication compliance. CM informed client that his assessment appointment with Laurie Garza, from LAMP VASH, was changed from 3/25 to 3/24. CM inquired if client attended all of his medical appointments at the VA. CM informed client that his six months in the program was approaching. CM informed client that an extension may be granted for him, only if client proves that he has a plan and is definitely working towards his housing goals.

R- Client presented well-groomed and with good hygiene. The client appeared calm and made good eye contact, was cooperative, and open for discussion. His affect was appropriate

Client has contacted Laurie Garza, from LAMP VASH, because he was told and concerned that he did not qualify for VASH. Client stated that Laurie Garza confirmed he does qualify for VASH, so he prefers working with LAMP VASH towards his housing goals, and with obtaining a VASH voucher. Client stated that he will attend his assessment appointment with Laurie Garza on 3/24.

Mental Health: Client stated that within the past month, his depression and anxiety levels have remained low, because he continues to take his psychotropic medications, and is staying active by attending both his medical and housing appointments. Client stated that he finds comfort with going to church in Little Tokyo, and plans on visiting the Angelus Plaza Senior Center when filming has completed. Client stated that he is much happier, knowing that he does qualify for VASH, and is excited about working with LAMP VASH towards his housing
