1. Explain why it is important to learn from incidents (guide: 50-100 words).

 Learning from the incidents is important to reduce the risk of repeating them. As we know any incident happens at a workplace cost the business a fortune and sometime human life as well but by investigating the incident we can find out the cause or trigger and then we can eliminate or minimise the risk of reoccurring of these incidents. Incident investigation also help us determine emergency response procedures and equipment to minimise the effect and loss caused by the incident.

Also, the great scientist Mr. Albert Einstein said;
“Anyone who has never made a mistake has never tried anything new.”

To determine the corrective actions after an incident, the first thing is to find is the source and different contributing factors, i.e. Design, Behavioural, Environmental, system etc. Then the risk level is define by keeping the likelihood and consequences in account. This will decides the priority of the correction action.
The implementation of corrective actions is very important step in the success in reducing the risk of incident at workplace. The implementation of the corrective actions either eliminate the risk or minimise the risk to acceptable level using hierarchy of controls. The most important and careful thing to keep in mind while implementing the new corrective action is effect of these changes on the health and safety. So
