Case #06CVD1620-2 Modification
This document shall serve as a Modification of the above mention divorce decree, agreeable by both parents: Kristen D. Lord (Mother) and Brian L. Lord (Father) concerning the residency and visitation of their two minor daughters; Sierra F. Lord (age 14) and Savannah G. Lord (age 12).
Residency
Both parents agree that Sierra F. Lord shall reside with her mother for 9th, 10th, 11th, and 12th grade school years. Likewise it is agreed that Savannah G. Lord shall also reside with her mother, but is limited to only her 7th & 8th grade school years. She shall return to live with her father for 9th, 10th, 11th and 12th grade school years.
At any time either parent feels it is in the best-interest of the children, they agree to make such arrangements to return either child to the other parent.
Visitation
It is also agreed that the Visitation Schedule also be Modified to reflect the following:
Odd Years:
Mother shall have Easter Weekend, First Half of Spring Break, Mother’s Day Weekend, First Half of Summer, 4th of July Weekend, Second Half of Christmas Break from December 26th at 10:00 a.m. until school starts.
Father shall have Second Half of Spring Break starting Wednesday by 6:00 p.m., Memorial Day Weekend, Fathers Day Weekend, Second Half of Summer Vacation, Halloween Weekend, Thanksgiving Vacation, and First Half of Christmas Break from the last day of school until December 26th at 10:00 a.m.
Even Years:
Mother shall have Second Half of Spring Break starting Wednesday by 6:00 p.m., Memorial Day Weekend, Mothers Day Weekend, Second Half of Summer Vacation, Halloween Weekend, Thanksgiving Vacation, and First Half of Christmas Break from the last day of school until December 26th at 10:00 a.m.
Father shall have Easter Weekend, First Half of Spring Break, Fathers Day Weekend, First Half of Summer, 4th of July Weekend, Second Half of Christmas Break from December 26th at 10:00 a.m. until school starts.
Both parents shall inform each other of any and all of children’s school/extracurricular activity schedules as they become available so each parent can plan to participate.
Taxes:
Both parents agree that the mother will continue to claim Sierra F. Lord and the father to claim Savannah G. Lord as dependants on their personal tax returns no matter where the children reside.
Medical / Dental
It is also noted that pursuant to the above case that both parents will notify the other of any medical, dental or optical procedure that either child may require and both be legally and financially responsible for any outstanding debt that may occur that is not covered by insurance until either child turns the age of 18.

All parties are in agreement, this 16th Day of June, 2013.
