Example Of Failurement Analysis
I am writing in regards to the failure to provide further appointments. There has been a failure to schedule further appointments since the last appointment dated 17th June 2016, during which I had informed you I was suffering a significant deterioration in my mental health.
This failure to schedule appropriate appointments has been ongoing; during a routine appointment with Dr. Dale dated 19th April 2016, you stated to Dr. Dale, you were unable to provide an update as you had not seen me.
Since that appointment, the only appointment was on 17th June 2016.
As my care coordinator, I feel it is important to inform you, I have not been seen by anybody since the meeting concerning evaluation for psychiatric hospital admission.
