The plan must involve written material as a method to help the congestive heart failure patient to recall the important details of self-care management. The educational written portion may also allow for the patient to have an area to record important numbers as to vital signs, and daily weights. The material need to include self-care tools and education for the congestive heart failure in an easily understood verbiage, and may need to in a secondary language to meet the needs of all patients. The education may be supplemented with other educational material such as face to face discussion, videos, and other resources. The nurse may also wish to emphasize the importance of the follow-up appointment upon discharge. The material should be used to assist the patient to begin to think about self-related health goals. Persons involved in creation of this educational written material included nursing, and nursing leadership that are involved in care of the congestive heart failure patient.
