E/M Nursing Codes: A Case Study
Assigning Evaluation and Management (E/M) Codes

o A 7-year-old is brought into the office unexpectedly by his mother on a very busy clinic day. He is coughing and wheezing. He has no known history of asthma, but frequently gets like this while playing soccer. He is in moderate distress. An H&P and physical exam is performed.

As there is no previous history of asthma within the patient the issue is an extended problem focused with E/M Code 99202 due to the fact the mother reports it happening every time he plays soccer so could be sports induced asthma but will need extended tests of the area and related areas to the issue at hand. Also this code represents new patients. Since there is nothing stated that he was a current patient at this clinic I am treating as a

Since such a thorough examination has taken place to find the extent of the problem the E/M code I would assign would be 99205 due to being comprehensive due to the wide range of tests and several key components are being done such as checking the detailed history of the patient and doing such a detailed examination of the patient.

o A 42-year old diabetic female presents with nasal stuffiness, a sinus headache, a sore throat and an unrelenting cough. You spend 12 minutes reviewing problems, diagnosing the patient as having sinusitis, and discussing treatment options. The patient is prescribed a 7-day course of the antibiotic Levaquin.

Since the patient is a diabetic she is more than likely a current patient that goes to the clinic on a regular basis. It is a straight forward examination that does not take much time, effort, or even tests in order to tell what is wrong and looks to be problem focused so the E/M code I would assign would be 99212

o Initial consultation for a 78-year-old woman with unexplained weight loss, abdominal pain, and rectal bleeding. Comprehensive history and examination

The difference between the official coding guidelines for using V codes in an inpatient and outpatient setting is the guidelines for an outpatient setting indicates code sequencing for physician office and clinical encounters. V codes also may be used as the principal and secondary diagnosis in the inpatient setting compared to those that may be as the first listed or secondary diagnosis in the outpatient setting. The most important difference in the official guidelines of V codes is that the definition of principal diagnosis applies only to inpatients in acute, short-term, general hospitals. This means that the V coding guidelines for inconclusive diagnoses were developed for inpatient reporting and do not apply to an outpatient setting. Diagnoses are often not established at the time of the initial outpatient encounter or visit, because of this, it is an extremely important guideline when using V codes. In many outpatient cases, the diagnosis V code for a presenting sign or symptom must be assigned because a definitive diagnosis has not yet been determined.…

This is a second Medicare compliance determination. The initial compliance determination was made by EHR which felt the patient should be observation status. Dr. Neilian disagreed. My clinical review of the chart reveals that the patient is 89-years of age, she presented with shaking, chills, fever of 103 and hematuria. Her medical history is significant for a previous history of right breast cancer, chronic acquired lymphedema, as well as hypertension. Her initial labs showed her to have an elevated white blood count and left shift, as well as a positive blood culture growing E coli. I believe that this patient represents acute inpatient admission. She requires IV antibiotics so I concur with the attending physician Dr. Neilan who believes…

T.B. is a 65-year-old retiree who is admitted to your unit from the emergency department (ED). On arrival you note that he is trembling and nearly doubled over with severe abdominal pain. T.B. indicates that he has severe pain in the right upper quadrant (RUQ) of his abdomen that radiates through to his mid-back as a deep, sharp boring pain. He is more comfortable walking or sitting bent forward rather than lying flat in bed. He admits to having had several similar bouts of abdominal pain in the last month, but “none as bad as this.” He feels nauseated but has not vomited, although he did vomit a week ago with a similar episode. T.B. experienced an acute onset of pain after eating fish and chips at a fast-food restaurant earlier today. He is not happy to be in the hospital and is grumpy that his…

The American Nurses Association (2008) suggests the use of 13 languages that nurses should to communicate thoughts, ideas, and care plans. Nurses work in ever specialty and are require interacting with each other and other specialties to benefit their patients. Recently my hospital has created the usage of an electronic interdisciplinary care plan (eIPOC). The eIPOC utilizes Nursing Diagnosis from NANDA International language to discuss patient problems and interventions being used to solve problems. This primary language allows nurses from the bedside to case managers to communicate patient progress. By having an established language, we all communicate within the same language making communication easy and efficient. Herdman et al. (2011) states that the NANDA International next editions will have significant additions for the usage by administrators, educators,…
