Age: 66 years old
Primary Diagnosis: Cellulitis to left leg
Secondary Diagnosis: Depression Osteoporosis Type 2 Diabetes Mellitus
Past Surgical History: Tonsillectomy- as child Cholecystectomy- 15 years ago (51 years old)
Current Medications: “migraine pills” St. John’s wort oil – for skin inflammation to left leg (for a week)

Past History: Smokes ½ a pack of cigarettes per day (since she was a teenager)
Weight: 70 kg
Height: 150 cm

DOCTOR’S ORDER:
Medications:
Ancef 750 mg IV q12h
Elavil 50 mg po qhs
Calcium Carbonate 1000 mg po once daily
Vit D 1000 units po once daily
Actonel 35 mg/week po
Vagifem 10mcg tablet into the vagina qhs
Imitrex 75 mg po q4h prn for a maximum of 2 doses daily
Metformin 750 mg po TID with meals
Buffered Aspirin 650 mg po q6h prn

QUESTIONS AND ANSWERS:
1. Why are these medications ordered for Ms. Sears?
What is the therapeutic effect of each medication?

Ancef (Cefazolin)- anti infectives, first-generation cephalosporins
Indication: It works by fighting bacteria in the body. Which is the cellulitis
(serious bacterial skin infection) in the case of Ms. Maggie.
Therapeutic Effect: Bactericidal action against susceptible bacteria.
Bind to bacterial cell wall membrane, causing cell death.
Elavil (Amitriptyline) - tricyclic antidepressants
Indication: used to treat symptoms of depression.
Therapeutic Effect: anti-depressant action.
Potentiates the effect of serotonin and norepinephrine in the CNS. Has an anticholinergic properties.
Calcium Carbonate (Calcium Salts)- mineral and electrolyte replacement
Indication: PO- Adjunct in the prevention of postmenopausal osteoporosis
Therapeutic Effect: Replacement of calcium in deficiency states.
Vitamin D (Calcifediol) -vitamins
