Good Morning Ms. Miller,
You are correct; Leslie will be working closely with the social workers in CPEP in referrals to our Bronx Health Home and connecting those patients already engage. As we agreed in our last conversation, I was waiting to see you in one of our weekly Clearinghouse meetings to integrate all the parties involved in this process.
In order to meet the new CPEP standard, will definitely instruct Leslie to actively work with the CPEP population and Social workers.
I think this is a great start in streamlining this process.
