Continuous Supportive Communication
Liam made moderate progress towards this goal. MHS has made Liam available for all schedule family visits with biological family. MHS provided transportation to each family court hearing in York county. MHS and MHP provide Liam with emotional support, MHS provides continuous supportive communication to Liam’s biological family and DSS. MHS continues to share Liam’s milestones and medical updates with biological family. MHP made monthly contact with the York County DSS worker for updates on Liam’s reunification. Liam will continue this goal.
