Abstract

Our child profile consists of a seven year old, first grade girl named Sophia Johnson. She was diagnosed with Emotional/Behavioral Disorder (EBD). Currently, she is reading at her grade level and is not received Title I services for reading. She is performing below her grade level in math, writing and spelling. Presently, she is receiving Title I services for Math. Sophia has been acting out in the classroom, on recess and during lunch. She has been acting out, crying inappropriately and having temper tantrums during lessons. This is a distraction to the other students who are trying to pay attention to the lessons. She also has difficulty sitting for long periods of time. During class, she will get out of her seat during lessons and walk around the room,

CLASSROOM/PROGRAM MODIFICATIONS

MODIFICATIONS NEEDED FOR THE STUDENT:

A. SUPPLEMENTARY AIDS AND SERVICES:

*Sophia will have a paraprofessional/aide to in the regular classroom to assist her on an "as needed" basis for when she is completing her work in math, writing and spelling.

*Her parent's also recommended that she be able to tape record the lesson, so she would be able to retain the information properly.

*Visual aids and overhead projections of the lesson will also be implemented during the lessons

B. INSTRUCTIONAL MODIFICATIONS OR SUPPORTS FOR SCHOOL PERSONNEL:

*When the paraprofessional in not in the room with Sophia, she needs to be seated up in the front of the room (preferably near the teacher's desk).

*She needs to be away from the door, because that is where she can get distracted easily. Classroom teacher and the paraprofessional will send home daily progress reports to be signed by her parents. This document will also stay in her permanent file at
