Case Study Of Maryann Harmon In Regal Heights Rehab Nursing Home
Hello,

I would like to address an issue regarding a long term custodial resident Maryann Harmon who has been in Regal Heights Rehab nursing home since 10/13/2015. I reached out to HRA/NHED providers relations to check the status of the case. I was informed by HRA that the case was denied due to the nursing home submitting a 259d (discharge alert). Even if it’s the member’s intention to return home, the case should still be processed within the guidelines of determining eligibility.

We would do everything necessary to coordinate a safe discharge if that’s what the member wants to pursue. However at the moment the resident is receiving custodial care, not skilled services.
It was not communicated to us that the member was being discharged

    At this time, a full overview of this case was given. Which is as follows: I explained the client is in Star plus waiver, a 30 day letter was issued from ASH on 1/6/17 and an MDU slot was requested on 1/6/17 and then again on 1/12/17 as I did not get a returned email that Cindy Pollan had already retired. Nor did I get a returned phone call from her voicemail, which was still active. After speaking with other DADS/HHSC contacts, I got your information and we spoke on 1/25/17. At the time, I explained that ASH did not want to hold the client until 3/1/17 due to no longer meeting medical necessity and why they issued the 30 day letter. This caused an issue, the client cannot return home, ASH needs to dischrge, yet he cannot enroll until 3/1/17 because of the Star plus waiver. I called seeking guidance on if the waiting until the 1st of the month was an HHSC rule that could be reviewed and changed for this special case or was the rule from Medicaid.…
