Case Study Of Ayden's Biological Family
Circumstance: Ayden will have supervised visits with his biological parents. Ms. Smalls (MHP) and Mrs. Wigfall (MHS) discuss Ayden’s schedule family visits.
Action: MHP and MHS discuss DSS schedule visit with Ayden’s biological family. MHS explain having Ayden ready and at the schedule location on time. Per MHS, Ayden’s mother and father was a no show. Ayden’s grandparents were at the location. MHS report giving the family current pictures of Ayden. MHS explained the DSS worker offered transportation to the visit after not seeing the Ayden’s parents at the location. MHP and MHS discuss the next schedule court date and alternative ways to encourage Ayden’s biological family.
Plan: MHP will provide support to MHS and continue to ensure Ayden

    At this time, a full overview of this case was given. Which is as follows: I explained the client is in Star plus waiver, a 30 day letter was issued from ASH on 1/6/17 and an MDU slot was requested on 1/6/17 and then again on 1/12/17 as I did not get a returned email that Cindy Pollan had already retired. Nor did I get a returned phone call from her voicemail, which was still active. After speaking with other DADS/HHSC contacts, I got your information and we spoke on 1/25/17. At the time, I explained that ASH did not want to hold the client until 3/1/17 due to no longer meeting medical necessity and why they issued the 30 day letter. This caused an issue, the client cannot return home, ASH needs to dischrge, yet he cannot enroll until 3/1/17 because of the Star plus waiver. I called seeking guidance on if the waiting until the 1st of the month was an HHSC rule that could be reviewed and changed for this special case or was the rule from Medicaid.…
