a. Earth Covered Magazine (ECM), bunker 3068.

1) No illumination. It has been noted that the magazine does not have interior lighting, due to no illumination the organization has to perform limited ammunition operations. (29 CFR 1926.56(a)) RAC 3=Moderate Recommendation: Submit a service order to have explosive proof ceiling light fixtures installed.

2) Facility maintenance. The magazine exterior security light sets does not shut off during daylight hours as designed due to faulty sensors. (AR 190-11 and DA Pam 385-64) RAC 3=Moderate Recommendation: Submit a service order to the faulty light sensor inspected, tested and replaced if needed.

Located on the shop floor, six fluorescent ceiling lamps were burnt out. (29 CFR 1926.596(a)) RAC 3 = Moderate Recommendation: Submit a service order to have the burnt out lamps replaced. c. Company area, bldg. 2914.

1) Slip/fall hazard. Stairwell step trends were very slippery due to laminated floor covering. (PL 91-596) RAC 3=Moderate Recommendation: Submit service order to have non slipped resistance strips installed to the step trends.

2) Observation. Normally an office desk chair life expectancy is eight to ten years, it was noticed that several offices (005; 006; 010; 022 and 023) desk chairs have exceeded their life expectancy (well over the ten year old). This can result in an increased risk of chair failure and a potential fall hazard to office personnel who are using these old chairs. This is a valid safety concern as this office has investigated several incidents in recent months where employees fell to the floor when their chair broke. 4) Slip and fall hazard. It was noted that both basement (male/female) shower floors does not have non-slip proof rubber matting for individuals exiting the shower after use. (29 CFR 1910.22(a)(2)) RAC 3 = Moderate Recommendation: (1) Post warning signs of the potential slippery floor. (2) Procure and install non-slip proof rubber matting or strips for the shower floors.

5) Observation. Located in the basement female latrine, it was noted that the toilet partition mounted to the wall was loose. Recommendation: Submit a service order to have the loose partition remounted to the

    At 2215 hrs, on November 28, 1942, Fire Alarm Headquarters from Box 1514, situated at Stuart and Carver streets, received an alarm.     The equipment could consist of wheelchairs, hoists, electrical equipment or even a bed. If something were to happen; for example, a carer not using a hoist correctly, then the service user could be at risk of developing bruises and minor or major cuts, in some extensive cases the patient could even end up being crushed by the hoist. Sometimes, it will not always be the carers fault, the hoist could be used without a carer knowing it is broken. The possible causes of a service user being hurt by a hoist could be because of the lack of maintenance and thorough examination or the patient could get caught between two different moving parts. This is a hazard for elderly people because when you get older, your bones become fragile and even the slightest fall could result in a major injury. To prevent this from happening, the Care Home should ensure that all equipment (even the least important) should have maintenance checks to make sure all equipment is fully working and even the slightest error with equipment should be checked over and fixed IMMEDIATELY. If their equipment is likely to break and get damaged then the Care Home should invest in some better quality hoists from somewhere that is a well known brand.
