Case Study: Bi-Weekly ILP Review
On 6/7/2016, CM met with the client for the first time since case was assigned to this worker on 6/2/2016. CM met with the client and completed Bi-Weekly ILP Review. In the meeting client appears to be cooperative and friendly. She was alert, satisfactorily groomed, and casually dressed. She was very loquacious. She made eye contact appropriately. Client’s mood was balanced and her affect was appropriate. She was oriented to person, place, time and situation. Client denied suicidal or homicidal ideation.

SOCIAL SUPPORT UPDATE: Client reported she gets along well with the residents in her dorm. At the present she didn’t have any issues or concern. CM inquired the reason client is non- compliant. Client stated most of the time she

SUBSTANCE ABUSE: Client continues to deny any past or present history.
MENTAL HEALTH: Client denies mental health but she reported the previous worker scheduled her appointment to meet with the onsite psychiatrist doctor but she was no show.

Client Rights and Client Code of Conduct Violations & Facility Rules Violations: Client was no compliant for face to face meetings. CM reiterates the Shelter Rules and Regulation and for the client to adhere to the 10pm curfew.

WORKER ASSESSMENT:. Client has been non-compliant with meeting for face to face meeting since the case was assigned to this worker. Based on this worker observation client appears to have the ability to identify and complete goal in a timely fashion. Client has the ability to live independently.

PLAN OF ACTION:
Client must maintain all scheduled appointments, client must attend HRA Friends Field interview scheduled for 6/8/2016, and if hired submit work scheduled, client must adhere to the shelter rule and regulation including the 10pm curfew, client must maintain dorm and self-clean at all time. CM reviewed the Bi-Weekly ILP Review. Client agreed and signed. Next Bi-Weekly ILP Review is scheduled for
