We have investigated your concerns regarding this issue. Our office contacted the office of Banner Physician Specialist of Arizona and spoke with Donna a billing specialist. Donna advised us that on date of service February 09, 2017 a Coumadin test was conducted as well as an evaluation by specialist Pradeep Agarwal, MD and medication Erythromycin 0.05% eye ointment was prescribed. Based on the information received you would be responsible for the $15 specialist copay. We are also unable to waive any cost sharing responsibilities such as the copay that was
