P: Timothy will reduce the overall frequency, intensity, and duration of anxiety so that daily functioning is not impaired as evidenced by increased participation in daily social activities.
A: MHP taught Timothy strategies for anxiety management. MHP taught Timothy calming skills. MHP assisted Timothy in identifying triggers that lead to anxiety and/or depression. MHP utilized techniques like reflection and role-play to assist Timothy in identifying fears, concerns, and anxiety. MHP assisted Timothy in confronting his self-disparaging remarks. MHP taught Timothy anxiety, problem-solving, and communication skills to reduce anxiety. MHP provided positive reinforcements.
