CLBC DAY PROGRAM QUARTERLY REPORT
AUG/SEPT/OCT 2013

Individual: Arren Nash
Keyworker: Sandy Hossack

Health Update

During this reporting period Arren has not had any physical heath issues to report. There were no toileting issues. Due to Arren’s increase in behaviors he was taken to a psch. Named Dr. Price on Sept. 11 and was given a new medication called Quetiapine. This was increased by 25mg on Oct.15/2013. Currently on 50mg.

Social, Recreation, and Community

Arren’s behaviors became extreme in nature during the month of august especially when he was having vacation time with family members. Due to Arren’s behaviors the social recreation and community time was very much limited.

Family involvement

Arren spent time with his family during the month of August. This was not a successful time due to Arren’s behaviors. Arren also has had few visits with his dad whom also has health issues.

General Behaviors

Arren has continued to need constant assistance with all aspect of his life due to his behaviors. These behaviors include an increase in aggression (hitting, pushing, pulling) towards his mom during transition times. Arren has also had a high level of aggression in public places which include throwing objects in stores attempts at breaking (hitting, kicking) glass windows at a bank and again at a haircutting establishment. There was also a high level of Arren’s typical behaviors which involve inappropriate touching of others, dropping to the ground, eating plants, issues with seatbelts in the car, scratching and picking at his skin. The most severe problem with Arren’s behaviors towards people when in the community becoming impossible for day staff to verbally and physical control. Arren was much better when given physical space and quiet time alone.

During the month of August day staff has Arren only six days due to vacation time. Arren received 8 PRNS. During the month of September Arren received 18 PRNS and the month of October Arren had 11 PRNS.
