2. (1) Mr P is a 59 year old with chronic back pain.
Using a care plan approach describe the nursing care for Mr. P.
Diagnosis: Chronic back pain.
Let Mr P know firstly that he will be assisted to deal with his pain so as to relieve any stress he may have associated with the back pain. Assess pain by using pain assessment scale 0-10.
Enquire as to the location of the pain; characteristics of the pain, how long he has had the pain for and factors that may trigger the pain.
Provide Mr P with the right sort of pain relief and administer early to provide immediate pain relief before it gets worse.
Educate Mr P on the use of other pain relief techniques beside medication and go through with him how to apply them correctly – i.e.; relaxation, massage, acupuncture, heat packs, hypnosis and music therapy.
Evaluate Mr P on the effectiveness of the pain relief.
Document the pain relief before and after administration to see if further action needs to be taken.
Refer to a physiotherapist.
Educate Mr P on any side effects that he may have from the medication and how he can manage it.
Make sure Mr P is moved correctly and repositioned at intervals.
Is he able to manage his pain independently?
Pain is decreased with medication and he uses the medication according to the prescription. Mr P has little or no side effects with medication.
He can manage life with as little pain as possible.

* Massage-reduces frequency of headaches and can improve quality of sleep * Regular gentle exercise-from walking to yoga-can reduce symptoms * 2 litres of water a day-keep body hydrated
* Myotherapy-releases muscle tension in your muscles by identifying trigger points * Physical therapy-helps with pain that originates in the muscles of joints of neck * Acupuncture-found to be helpful for headaches with the insertion of fine needles into certain areas of the skin * Stress management-
* Relaxation techniques-deep breathing, meditation-manage stress * Tai chi-manages stress and helps relax
* Bioelectric therapy-better cope with pain and stress by altering behaviour, thoughts and feelings * Hypnosis-helps manage stress and increases relaxation to cope with symptoms * Herbs, vitamins and minerals-some evidence that they may improve migraines
