• Describe the name of the owner of the project/business. If a partnership or cooperative, the name of the officers and key personnel, and their capacity to put up the business

2. Proposed Name of the Project/Business
• State the name of the business (that will be reflected in the business name registration/business permit) • Explain the reason/s for your choice of name

3. Type of Business Organization
• State the type of ownership (sole proprietorship, corporation, partnership or cooperative) • Explain the reasons for your choice of name

4. Location of Business (Office, Store, Factory, or Farm)
• Briefly discuss advantages and reasons for the choice of location
