Preoperative Diagnosis: Recurrent left inguinal hernia
Postoperative Diagnosis: Recurrent left inguinal hernia

Operative Procedures

Procedure: Repair of recurrent left inguinal hernia with Ethicon mesh plug.

Procedure Description: The patient was given a general anesthetic. She had SCDs on, which was functional. She was prepped and drapped in supine fashion. A skin incision was made in the left inguinal region. We did not go through the previous incision. We just went superior and medial to this. We went down to the external oblique and cleaned it off. We then opened up the external oblique along the direction of its fibers. We identified a good size indirect inguinal hernia. The direct space actually was well maintained. I then was able to do a high dissection of this moderate size left indirect inguinal hernia. I then reduce this. I use media Ethicom prolene hernia system mesh plug. We placed it underneath the conjoined tendon and sutured the conjoined tendon superiorly. We sutured from conjoined tendon down to the circular part of the mesh and then brought it back up. We did the same medially, making sure that we stayed away from the deep inferior epigastric vessels and then we went inferiorly down from the reflected part of the inguinal ligament. We then put the second part of the mesh down on the direct space. We sutured it to pubis medially,...
